and corresponding instructions. Once you have read the instructions
and completed the form, send in a complete package with any required
attachments and checks. If checks are involved use mail or overnight
only. All forms, attachments and checks for a particular order must
come in as one complete package.

1) Loan Servicing Checklist
Use this checklist as a guide to put together a complete Loan Servicing Package for submission. Read each item carefully and include this form in the complete package sent in. Remember, we can only accept and set up loans that comes in as one complete package per loan (unless this is a bulk transfer using our Loan Board Template). To get this form please

2) Loan Servicing Setup Form
To create a Loan Servicing Setup Package and start servicing on a NEW LOAN, completely fill out, print and then submit this form for each investor on the loan, along with (1) Loan Servicing Checklist, (2) copy of the Note, (3) copy of the Deed/Mortgage, (4) copy of any Assignments, (5) any odd day's interest check, or (6) any prepayment checks. To transfer servicing on an EXISTING LOAN also include a copy of (7) RESPA "goodbye" letter, (8) any Loan Modifications,
and (9) the Payment History. Include a check for $25 per Standard loan, or $150 per performing Bankruptcy or Forbearance Plan loan, for the one time Loan Setup Fee. To get this form please

3) RESPA "Goodbye" Letter
This form is sent to the
borrower by the current Servicer to transfer servicing on an
existing loan. It is not needed to start servicing a
new loan. Date and send the letter at least 15 days before
the Transfer of Servicing Date. Transfer of Servicing Date must be
at least 15 days after the date of the letter. Use the same date in
each "Transfer Date" field. Make the obvious changes in the
active fields. Regardless of the Transfer of Servicing Date in the RESPA letter, consider the loan transferred when it is sent to FCI. Any borrower payments received once the package has been sent to
FCI must be endorsed and forwarded to FCI for processing. To get

4) TILA Ownership Transfer Notice (for transferring full or partial ownership)
Effective May 20, 2009 when an existing loan secured by the principal dwelling of a consumer is purchased or assigned
in full or part, the new Investor/Lender (Creditor) must notify the
borrower of the transfer of ownership within 30 days. Use this
checklist to provide the information needed for FCI to send out the
Truth-In-Lending Act Ownership Transfer Notice. We can only do this Notice for loans in our Loan Servicing system. Include a $25 processing fee when submitting the completely filled out checklist. To get this form please CLICK HERE.
5) Loan Servicing Direct Deposit Form
This form is used to set up the automatic deposit (ACH) of funds
for Note Rate/Sold Rate or Broker Servicing Fee from loan payments into a Broker or Investor/Lender's bank account. This form is
not needed for an Investor whose banking information is included on the Loan Collection Setup Form.

6) Bankruptcy Information Form
Provide a copy of the following: Promissory Note, Deed of Trust, any modifications and any assignments along with account history and foreclosure documents (if foreclosure is in progress). Include the $100 Bankruptcy Relief of Stay set up, tracking and coordination fee. To get this form please

7) Eviction/Unlawful Detainer Information Form
Forward copy of Trustee's Deed upon sale, occupant's name(s), and property address.

8) Request to market REO Property Form
Forward copies of Trustee's/Sheriff's Deed Upon Sale, last Appraisal, and any Broker packages, property valuations or property inspections, Hazard and Liability Insurance related to this property.

9) Request for Full or Partial Reconveyance
Submit this completed form along with the original Note, Deed of Trust and a Substitution of Trustee form. Include $60.00 to cover the standard $45.00 reconveyance fee plus the $15.00 recording fee.

10) Substitution of Trustee for CA and NV
This form is used for Foreclosure and Reconveyance purposes.

11) Start Foreclosure: CA and NV
To
start a California or Nevada foreclosure, submit this completed form
along with a copy of the Note, Deed, any Assignments, and any
Modifications.
Also include a Substitution of Trustee and either the CA
Beneficiary Declaration of Compliance or the CA Beneficiary
Authorization. A deposit is required to start a foreclosure. Call
extension 206 for an estimate of total foreclosure costs and the

12) CA Beneficiary Declaration of Compliance
The
Declaration of Compliance with California Civil Code 2923.5 (Senate
Bill SB 1137 effective September 6, 2008) must be filled out and
signed by the beneficiary (Investor/Lender) or agent and submitted
with the Request to Start Foreclosure in CA. To get this form please

Beneficiary Authorization form appointing FCI as your agent to
handle the compliance requirements. (FCI will sign the Declaration
of Compliance).
13) CA Beneficiary Authorization
This
form appoints FCI as agent to handle the California Civil Code
2923.5 (Senate Bill SB 1137 effective September 6, 2008) compliance
requirements and sign the Declaration of Compliance. The fee is $150

14) Start Foreclosure: National (except CA
and NV) To start a foreclosure, submit this completed form along with a copy of the Note, Deed/Mortgage, any Assignments, and any Modifications. There is a $175 set up, tracking and coordination fee plus attorney costs. A deposit is required to start a foreclosure. Call extension 287 for an estimate of total foreclosure costs and the deposit amount. In the event the borrower files bankruptcy during foreclosure, there is a $100 Bankruptcy Relief of Stay set up, tracking and coordination fee and additional attorney costs.
