This form is used for Foreclosure and Reconveyance purposes.

To start a California or Nevada foreclosure, submit this completed form along with a copy of the Note, Deed, any Assignments, and any Modifications. Also include a Substitution of Trustee and either the CA Beneficiary Declaration of Compliance or the CA Beneficiary Authorization. A deposit is required to start a foreclosure. Call extension 206 for an estimate of total foreclosure costs and the deposit amount.

The Declaration of Compliance with California Civil Code 2923.5 (Senate Bill SB 1137 effective September 6, 2008) must be filled out and signed by the beneficiary (Investor/Lender) or agent and submitted with the Request to Start Foreclosure in CA. Do not submit this form if you are submitting the CA Beneficiary Authorization form appointing FCI as your agent to handle the compliance requirements. (FCI will sign the Declaration of Compliance).

This form appoints FCI as agent to handle the California Civil Code 2923.5 (Senate Bill SB 1137 effective September 6, 2008) compliance requirements and sign the Declaration of Compliance. The fee is $300 per loan.
